SCM Advance Care Planning (ACP) Alert

The SCM Advance Care Planning Alert will improve our ability to alert staff in our Emergency departments and on admission to hospital about the existence of patients’ wishes and/or ACP documents stored in the Advance Care Planning (patient level) folder in Chartscan. Conversations about treatment choices around intubation, resuscitation, etc., or substitute decision maker preferences, often take place in our out-patient clinics or during hospital admission, but we have had difficulty ensuring these advance care planning wishes are available for future admission.

The procedure for setting the alert is being communicated directly to the Unit Coordinators by Deborah Shaw. An IMIS announcement will be sent out next week describing the procedure (see attached) and an announcement will appear in PHC News on July 12th.

There is much happening to encourage ACP in British Columbia, and our procedures at Providence provide important support to the Advance Care Planning process.

Education is available to help encourage ACP within clinics and programs and to develop best practices in each area:

ACP skills workshops and in-services can be provided in your clinical area – contact wrobinson@providencehealth.bc.ca

  • An online ACP education module is in development and lunch and learns will be scheduled beginning in the Fall (an update on this education rollout will be presented at ELF in September)
  • Patient/family education sessions are now scheduled monthly (the 3rd Wednesday of each month from 1:15 to 2:15 pm in Hurlburt auditorium).
