To All Medical Staff
A reminder the influenza season is upon us.
As of December 01, 2012, all staff at PHC must have either received the flu vaccine or be prepared to wear a mask in all patient care areas until March 31, 2012.
You may obtain your free flu shot at the many staff flu clinics being offered at PHC, or any of the Health Authorities. For PHC flu clinic times and locations, please see the Flu Clinic Schedule . Free flu shots are also available at community pharmacies, public health clinics, and doctors’ offices (with appropriate ID).
Reporting and Accountability
If you receive a flu shot at a PHC clinic, you will receive a sticker to be placed on the lower right corner of your ID badge, indicating that you are immunized. For staff receiving their immunization outside of PHC, you should report your immunization via e-mail to: firstname.lastname@example.orgPlease include your name, date, and the location where you received your flu shot.
You may also request a flu sticker via e-mail to the PHCfluclinic address, or alternately self-report at a PHC flu clinic and request a sticker.
To date, approximately 200 of our Medical Staff have confirmed receiving the vaccine.
