The VCH/PHC regional Heart Failure Strategy in conjunction with the Clinical Guidelines Initiative is working towards improving HF management in both acute and community settings. Heart Failure (HF) is a leading cause for hospital admission and there are a number of known gaps in care for patients with HF. Over the past months the HF strategy has been working with professionals and teams at Providence Health Care to institute guideline driven care for heart failure patients.
Starting June 4, 2012 PHC will go live with the new regional Heart Failure pre-printed order package. This is the first step of the implementation phase.

Next steps in this initiative will be to educate staff on HF management based on Canadian HF guidelines. Education sessions will target physicians, nurses, allied health, and administrative support staff.
